Concussion Resources & Forms
Student Athletic Physicals Required
In accordance with Nebraska School Activities Association (NSAA) guidelines, all Nebraska high school athletes participating in interscholastic sports are required to have a physical examination. The physical must be completed after May 1st of the current year to be valid for the upcoming school year. This requirement helps ensure that all student-athletes are medically cleared and fit to participate in athletic activities.

NFHS Medical Release Form: Wrestler Skin Lesions
This form, developed by the National Federation of State High School Associations (NFHS) Sports Medicine Advisory Committee, is used to determine whether a wrestler with a skin lesion is medically cleared to participate in competition. It provides guidelines for physicians to assess the risk of contagion and ensure that the wrestler is no longer infectious.
While the form does not establish a national medical standard, it reflects expert consensus and research on communicability and return-to-play decisions. It is intended to support consistent, informed decisions by medical professionals, helping to protect the health and safety of all athletes.
